Role Overview & Responsibilities
We are seeking to recruit a new team of two individuals with a passion for helping children, young people and their families to get the right support from services across the whole system.
Two Dynamic Support Keyworkers employed by NHS Cheshire and Merseyside ICB, Warrington Place and work across Health, Social Care, Education and other settings to coordinate and provide challenge to the system, thereby ensuring that children/young people and their families can access the right support to enable them to achieve better outcomes.
The two Keyworkers will work with another Keyworker from Halton and will work across both areas.
This new way of working will be targeted to support children and young people with Autism and/or Learning Disabilities who are mental health inpatients or at risk of being admitted, including those who face multiple vulnerabilities such as being looked after.
