Role Overview & Responsibilities
You will have a keen interest in the field of intermediate care and rehabilitation. You will provide valuable support to patients who have been admitted for rehabilitation following admission to hospital or who have been referred for consideration of admission avoidance and act as a role model to staff working outside of the expert team
You must possess excellent interpersonal skills as well as the drive, vision and initiative to motivate others and meet the demands of the service. You will be experienced at maintaining patient records and databases and have knowledge of the intermediate care.
This post is a permanent post and to ensure that the requirements of the service are met, is full time. The service operates 7 days per week and therefore this role may require weekend working
